TESA vs PESA: Which Is Better for Male Infertility?

When a man’s semen test shows no sperm, it brings a lot of fear. Most couples feel lost and think pregnancy won’t be possible. But that’s not true. Medicine has come a long way. Understanding Why These Procedures Are Needed

What Is TESA
TESA means Testicular Sperm Aspiration. It’s a small and simple procedure. Local anesthesia is given, so there’s no pain. With a thin needle, the doctor takes a small bit of tissue from the testis and checks it under a microscope. Sperm found in that tissue are then used later for treatment.
TESA helps when sperm production is very low or blocked inside the testis. The process is quick, safe, and men can go home the same day.
What Is PESA
PESA means Percutaneous Epididymal Sperm Aspiration. It’s also done under local anesthesia. Here, the sperm are taken from a tiny tube near the testis called the epididymis, where sperm are stored after they are made.
This is used when sperm are being produced normally but can’t come out because of a blockage. It’s a quick process, doesn’t need stitches, and recovery is easy.
How TESA and PESA Differ
TESA and PESA look similar but are done in different places. In TESA, sperm come directly from the testis. In PESA, sperm come from the tube beside it. TESA is helpful when sperm production is low, and PESA works well when sperm are fine but stuck because of a blockage.
Both are short, safe, and almost painless. The doctor chooses which one fits best after checking reports, hormone levels, and male infertility causes.
Which One Is Better
There’s no fixed answer. Each man’s situation is different. For some, PESA gives enough sperm for treatment. For others, TESA works better. The aim is always to collect healthy sperm that can be used for pregnancy.
