From Two Failed IVF ICSI Cycles From Elsewhere to Success at Sunflower Women’s Hospital : An NRI Couple’s Journey Through Severe OATS & High DFI

A couple from Australia, married for seven years, had been struggling with primary infertility. After undergoing two unsuccessful IVF cycles abroad, they arrived at Sunflower Women’s Hospital, searching for answers.

Upon evaluation, the husband was diagnosed with severe Oligoasthenoteratozoospermia (OATS) and high FSH levels. Further investigation revealed a sperm DNA fragmentation index (DFI) of 74%, a key factor in their previous IVF failures. To improve sperm quality, he was started on antioxidant therapy, and we decided to proceed with Testicular Sperm Aspiration (TESA) as a source of sperm for their next cycle.

On the day of the wife’s egg retrieval, TESA was performed, and ICSI was carried out using testicular sperm. This approach led to the formation of high-quality blastocysts, which were cryopreserved for a future transfer. Before proceeding, we conducted a pre-IVF hysteroscopy to optimize the uterine environment.

In the following menstrual cycle, two frozen-thawed blastocysts were transferred. This time, the outcome was different—the couple received the long-awaited positive pregnancy result on the first attempt at our center.

Today, they are happily expecting, with a healthy 33-week pregnancy progressing well. Their case highlights the importance of detailed diagnosis, personalized treatment strategies, and advanced reproductive techniques—all of which are available at Sunflower Women’s Hospital, ensuring that even the most challenging infertility cases can be successfully treated with the right approach.
