A Triumph of Hope: Achieving a Healthy Pregnancy Against All Odds

A 39-year-old woman, married for 15 years, arrived at Sunflower Women’s Hospital with a profound desire to overcome her challenges and complete her family. Her journey was one of resilience, marked by a history of secondary infertility and a 15-year-old son living with Duchenne muscular dystrophy (DMD).

The couple sought a ray of hope, and after an in-depth evaluation, her husband was diagnosed with mild oligoasthenoteratozoospermia, though he tested negative for thalassemia minor. The wife’s blood reports were normal, but her AMH level was low at 0.9 ng/ml, indicating decreased ovarian reserve. She was also identified as a carrier of the sickle cell trait, and her ultrasound revealed an adenomyotic uterus with ovaries showing only 2-3 antral follicles. Genetic counseling confirmed that DMD, an X-linked recessive disorder, was the key genetic challenge.

Further testing pinpointed the dystrophin (DMD) gene as the culprit, and following the geneticist’s guidance, we recommended IVF with their own eggs and sperm, incorporating preimplantation genetic testing for monogenic diseases (PGT-M) and aneuploidy (PGT-A).

Her journey began with stimulation using recombinant FSH and LH. During the first cycle, ICSI yielded two embryos, which were frozen. Due to poor embryo formation, the couple was counseled for dual stimulation. In the second cycle, ICSI produced three more embryos. PGT-M/A testing was performed on all five embryos, revealing that two were genetically normal while three were abnormal.

A single healthy embryo was carefully selected and transferred during a frozen embryo transfer (FET) cycle. To everyone’s delight, the patient conceived on the first attempt. Subsequent prenatal testing, including NIPT and amniocentesis, confirmed normal results, alleviating any lingering concerns.

At 39 weeks, the couple’s hopes were realized with the arrival of a healthy baby boy. Their long journey of determination and trust in advanced medical care had finally culminated in the gift of a perfect new life, bringing joy and completeness to their family.
