A 36-year-old woman, married for 16 years, approached us after experiencing 18 failed IVF cycles at reputable centers in Ahmedabad, 10-15 unsuccessful IUI cycles, two laparoscopies, and five failed self-embryo transfer cycles, as well as 13 failed ovum donation cycles. Both she and her husband had high BMIs, and she had a history of irregular periods while her husband had severe oligoasthenoteratozoospermia. The emotional toll was immense, and when she asked if we could help her achieve pregnancy, we felt the weight of her hope and determination.

We conducted thorough investigations, including blood and hormone tests, ABDO-PELVIS ultrasound for her, and blood tests, hormone analysis, semen analysis, sperm DNA fragmentation, and karyotype reports for her husband. Her results showed polycystic ovaries, a small uterus with a thin endometrium, and normal blood and hormone levels. Her husband had severe OTA, normal FSH, decreased testosterone levels, and a high DNA fragmentation index, but his blood and karyotype reports were normal.

The couple received counseling on diet and lifestyle modifications and started medical treatments for two months. The husband was given antioxidants, human chorionic gonadotropin injections, and multivitamins to improve sperm count. The wife was given estradiol valerate for 10 days to assess her endometrium, followed by a pre-IVF hysteroscopy.

We proceeded with an ovum donation cycle using TESA (Testicular Sperm Aspiration) due to the high DNA fragmentation index in the husband’s sperm. To prepare the wife’s endometrium, we used a hormone replacement cycle with a high dose of estradiol valerate, antioxidants, vitamin E supplements, intrauterine G-CSF, and low-dose corticosteroids for 20 days. Once her endometrium reached a satisfactory 8.7mm triple line, we transferred two grade 1 blastocysts.

Post-embryo transfer, she received high doses of estradiol valerate, oral, intravaginal, and injectable progesterone, low-dose corticosteroids, LMWH, and antioxidant support. The patient conceived on the first attempt and still has eight embryos cryopreserved with us.

The couple was overjoyed and could hardly believe that their 16-year journey to achieve a pregnancy had finally come to a joyful conclusion with the birth of a healthy baby girl.
